What is a Consent Form and Jotform?
We use an electronic consent form for accessibility, security, and user experience. Once you complete the form, we will follow up with you when we visit your kid's school.
We use a third party known as Jotform which offers All Smiles HIPAA compliance, offering several benefits for our dental hygienist handling sensitive patient data.
School Districts We Visit
All Smiles provides free dental services to K-8 schools in the Portland Metro Area. Each school year, All Smiles visits schools weekly and during the summer.

What free services does All Smiles provide?
- Dental screening: Our dental professional looks in the student’s mouth to check the health of teeth and gums.
- Dental sealants: Coatings may be put on the student’s back teeth to prevent cavities.
- Fluoride: Brushed on teeth, this vitamin makes teeth stronger and prevents cavities.
